You can get a copy of your record from the church where you were baptized

They will not be upset at all, the following process will be implemented:

Conditional Administration of the Sacraments
Since the sacraments of Baptism, Confirmation and Holy Orders imprint a permanent character, they cannot be conferred more than once. But, for various reasons (Was the proper form employed? Was the minister duly authorized?), doubts may arise as to whether one of these sacraments has been truly and validly conferred in the first place. If investigation cannot resolve such doubts, then these sacraments can be conferred “conditionally,” i.e., on the condition that the sacraments were not conferred on the occasion concerning which a legitimate doubt exists.

Just go to your old church, call or email them. AND try to get them. RCIA is a good course, went though it last year.

Or just try to remember when and where it was done.

You can get your Baptism certificate from the church you were baptized.

Give a call or email and one will be on its way to you.
