It seems that no-one thought that the person, even though old and ill, might just have been joking. How would the asker know? His/her grasp of English is weak, after all. Do you all really think that old people with health problems can't tell the difference between an actual piano and a much smaller picture of one on the wall? Shame on you all! Go and stand in the corner facing the wall, and make sure you don't tilt the picture while you're there.

Tell him(her)that it's just a lovely picture and keep making friendly conversation. Ask them about significant events that may pertain to this elderly person. Like if he served in a war, worked as a teacher, truck driver anything that he may be proud of having accomplished. They love to be reminded about their successful past. I did lots of volunteer work at a nursing home and it was inspiring hearing about their vast experiences and knowledge.

I would start by asking questions like" do you like music?" and "what somgs are your favorite?" that way you dont have to tell the old person that the piano isnt real, its just a picture. old people are often sensitive about things when they get dementia.

you don't answer, you ask.
ask the person if they played piano, let them share their memories with you, it's just as good as playing the piano yourself.
if you can't play piano then help them remember the piano.
put a smile on that face.

get an audio tape of piano playing and pretend you are playing the picture

tell him it is picture,then maby ask him does someone he knows play the piano and talk to him about that
