Worrying about keeping it real means you're concerned that people may perceive you as a fraud or faker. People who keep it real don't have the time in the day to worry if they're a fraud. Worrying about that is the first step to not keeping it real.

"Keeping it real", when applied to railroading, as questions are in this category, is a survival technique.
When one crests a grade with between 10,000 and 14,000 tons in tow, and that slack comes in and boots you in the butt, you are very much in the here and now, and that is as real as it's going to get.
