First-time sex isn't normally painful for men. Unless you're talking about the first time the man receives it anally, of course.

only if the vagina is not lubricated, then it can be extremely painful for both people (i've heard of this happening most commonly during sex in a swimming pool because of the bleach in the water.) to avoid this, do 20-30 minutes of foreplay or get a commercial lubricant.

Yes this is when their foreskin tears away exposing the head of their penis for the first time. It is very painful so be nice and comforting to him.
