He has many. One of the most obvious problems is called body dismorphic disorder. Body Dysmorphic Disorder (BDD) is a mental disorder, which involves a disturbed body image. Body dysmorphic disorder is generally diagnosed of those who are extremely critical of their physique or self image, despite the fact there may be no noticeable disfigurement or defect;
That is the reason for all of the plastic surgeries he has had. People with this disorder are never satisfied with the results of plastic surgery. (hence the repeated surgeries) Normally a plastic surgeon will not touch someone with this problem, but money talks.

His fans may get angry, but it's true, he's mentally unstable. I mean, imagine being one of the most famous entertainers of all time ,and losing a childhood to fame. Yeah, you may lose a few screws up there in the head.
