Pick it up through it in the trash wash your hands with soap wash your hands 4 times and your hands will be clean or just live the dirty napkin on the ground or burn the napkin.

I would pick it up. If I had a napkin on my lap in the first place, that is. Usually I just wipe my mouth on my sleeve.

That would depend on where I was... If I was at home with the family, I would reach down and pick it up.. If I was at a more formal dinner, I would just leave it on the floor.... However, just prior to leaving the table.. I might reach down and pick it up and place it on the table.

depending on where i am i reach nad get it but usually my husband and i ask for extra napkins before we start in a fine dining though i will ask our server for another napkin

At home just pick it up, shake it out and vacuum the next day. At a formal get up to excuse yourself and notice it, pick it up and put it on your chair. With friends just use your hand afterward... . . .

You are supposed to leave it on the floor and ask for another. If you want, when you finish eating and are ready to leave the table, you can pick it up and place it on your chair.
