I don't know, this is a good question, but I would like to try it. When I've heard of the word "girlfriend", I always think of someone who's gonna be there for you in a committed relationship. She's gonna be with you through thick and thin, despite all the hardships that you face. It's the person you connect with on a deeper level than anyone else, someone who you want to spend quality time with, every chance you get. Usually a girlfriend is the person who you're closest to, can share anything with, and is the person you're intimate with as well. Of course, today's society changes everything and the definition of "girlfriend" is being thrown all over the place, but that's always the way I've seen it to be.

It's difficult to say. There's a certain level of commitment, but usually not love.
Let's say some requirements. Mutual Romantic attraction. Mutual exculsivity. She's a girl. Compatible personalities.
Usually there is some degree of public displays of affection (PDAs) ranging from hand holding to full fleged make out sessions.
Girlfriend, and boyfriend as well are both rather juvenile terms. They aren't usually used for serious relationships (romantic partner might be more suitable) or for people over 30.

Spending a lot of time together, having a friendship that goes deeper than average. Making each other laugh. Always wanting to know what the other is doing, not to check up just - are you having a good day, to share something funny.
you could throw sex in, but you don't have to.

If a straight guy says it, it means the girl he's exclusively dating.
If a straight girl says it, it means her close female friend.
If a gay guy says it, it means his close female friend.
If a lesbian says it, it means the girl she's exclusively dating.
If someone greets you with it on the street, it's an effusive way of saying "hello!" (unless you're a guy, then if someone greets you on the street with it, it means the greeter is gay)
Gee, wasn't all this simpler back in 1930 or so?

Well, if you're a girl, then your friends are your girlfriends.
If your a guy, then she goes out with you, and you go to the malt shop and hang out with Fonzi and Richie Cunningham, etc.. and then later you go to inspiration point (just the two of you) and make out and stuff.

if a girl refers to another girl as her girl friend, then usually it is just her friend that is a girl. but if a guy refers to a girl as his girlfriend, then usually they are humping. there are exceptions, though. as in homosexual men and women, you get the idea.
