Those nations that poses the missile technology is a very limited club. Suppose Iran develops a nuke. They still need some way of delivery.
It doesn't really matter who goes first, the chances that there will be a chain reaction between nations/groups that will cause mass disstuction on a world scale.

The US dropped 2 atom bombs on Hiroshima and Nagasaki in Japan, although Japan was already defeated.
Either they or Israel on their behalf will fire the next ones as well.

probably the good old USA, someone else will probably set off the 1st nuclear device in some city, but it will be the US reacting militarily.

However some terrorist will probably eventually detonate one that they have driven or floated into another country.
