Turns out the recipes are kinda varied and the term stands for a mixed drink that is served hot. There are many variations; the essential elements are brandy, rum, or bourbon, spice, sweetener, hot water, and citrus.

Here are some versions:
Take one lump of sugar; dissolve it in a little hot water in a mug; add a piece of cinnamon, a lemon twist, four cloves, and an ounce of whiskey; and then fill the mug with hot water.
(You can use molasses or honey instead of sugar, rum or brandy can substitute for whiskey, and allspice and nutmeg are other possible flavors to put in the mix.)

A hot totty feels wonderful on a sore throat. I still drink them today when I'm not feeling so good.
I use a good black tea like Morning Thunder or English Breakfast and let it steep for about 3 minutes. Then mix in a tablespoon of honey, squeeze of a lemon wedge and half a jigger of good Ky Bourbon (Knob Hill, Makers Mark or Woodford Reserve).

My mother had something like that. What it was called I don't know. just that it worked.
Ingredients: Sugar, lemon, candy cane, honey, hot water.
How to: Boil the water until it starts to bubble. Pour into a cup. Add 2 ample slices of lemon. Then put on a hunk of candy cane, not the whole cane unless very small. 5 to 7 heaping table spoons of sugar. Then a table spoon of honey. Swig it down as fast and as hot as you can stand it. Then get under a comforter, and or 3 to 5 heavy blankets. By morning you will feel 80% better. For those who want more of a kick, you can add rum, Bourbon, gin, or Cognac to the brew.
