No....NOT usually. It depends on who your health insurance is through, and what they require. You need to check with them. Anymore, a lot of insurance companies have dropped the PCP referral needed to see a Specialist in most cases, but I'd check to see what your specific plan requires to be sure so you don't get stuck paying out of pocket.

Only if you want your insurance to pay for it. If that is not an issue(think paying in cash), your sports physician will normally see you (after you set up an appointment).
