do they have a website, you should be able to request the forms off their website. do a search for florida department of transportation and you should get the the website no problem

go to the website for the state of Florida and there should be info on various state agencies.

DMV should have all records...Division of Motor Vehicles...if not try the CLERK OF COURTS in that county
