think of a gromet like this.
the eurotunnel is being built, but all the time the area that has just been dug out is filling itself up again. so we put concrete all around the tunnel to stop it filling in.
the gromet is like the concrete in the tunnel. it is like a very tiny straw that is inserted into the ear canal to keep the passageway open.
I had them 3 times as a child, it is a very simple and quick procedure and i should imagine the worst you would experience is a little earache just after you wake up.
why did i have them 3 times? Well they dont last forever. a couple of years i think is the average lifetime, however, once is usually enough for most people (my ear canals are amazingly small though, unlike most patients')

grommets? i can't imagine why anyone would have to go to the hospital to have grommets put in their ears. here, they go to piercing shops, and voluntarily, at that. still, grommets may be another word for what we, colloquially, i suspect, call, "tubes", here. i had tubes in my ears, as a child, because my ear canals would have closed up, without them. it's a simple procedure, which can be done in a doctor's office, actually, and the worst part of it, for me (i went to the hospital), was the nausea from the general anaesthetic, afterward. if this is the procedure you're talking about, i don't see how it would be any worse for an adult, than it is for a child.

I had them done when I was a child.
Relatively straight-forward.
The only thing I can really remember about it was not being able to go swimming throughout the entire summer holidays.

I doubt it is any worse being older. Should be fine. Don't worry!
