According to the newest edition of Wedding Etiquette by Emily Post, the bride should wear the engagement ring on the RIGHT hand on the day of the ceremony. This way the wedding band can be placed immediately on the LEFT finger during the ceremony.
Alternately, a bride can wear the engagement ring on her left middle or pinky finger - but there's a chance of it getting stuck or slipping off.
A final alternative might be to have the bride slip her engagement ring off JUST PRIOR to the ring exchange - but this can offer the chance for a loss/mishap.
I wore my engagement ring on my right hand for the ceremony. NO ONE saw or noticed the difference, and directly after the ceremony I quietly slipped the engagement ring onto my left hand - and no one was the wiser.

At the wedding ceremony, the ring is usually held by the best man until required for the service, or tied to a pillow held by a ring bearer. The bride should remove her engagement ring for the ceremony (maybe wear it on the ring finger of her other hand), so that the wedding ring may be slipped onto her left ring finger by the groom. Once the ceremony is over, the engagement ring may be put back on without removing the wedding ring. The idea is that the wedding ring is closest to her heart.
Another thing we do is the bride if possible wears her engagement ring on her left hand either on her middle finger or index finger. Usually the flowers covers them anyway. Then casually after the wedding band is placed on and they are facing me during the last portion of the vows the brides have slipped their engagement ring back onto their proper finger and they feel more at ease with both rings being on the correct finger in the correct position.

Don't have a ring on the hand you want the wedding band on. If you want, put your engagement ring on the other hand. Then, after the ceremony, you can put the engagement ring over top of the wedding band if you want.

What I did was wear my engagement ring on my right hand during the ceremony, and then afterwards just switched it over to my left hand. The wedding band was on the inside.

The wedding band goes on the inside, because it is closest to your heart. Nothing is to come between your marriage "the ring" and your heart. During the ceremony, either wear your engagement ring on your right hand, or have your maid of honor wear it. Slip it back over after you've exchanged rings. Or, you can have whoever is carrying the wedding rings, the best man or ring bearer, hold your engagement ring also, and have your soon-to-be husband put them both on your finger at the same time.

Don't wear it during the ceremony. Put it on after your vows. I almost lost mine, taking it off to put on my wedding band. The wedding band goes on before the engagement ring, and after the ceremony. And, by the way, congratulations.
