The ingredients are Malibu Rum, Creme de Banana, Midori, Sour Mix and Pineapple.

Any idea on how much of each and how to prepare the drink?

These are the closest I could find for you:

Coppertone Punch (no sour mix)

* 3/4 oz Creme de Banana
* 3/4 oz Malibu rum
* 3 oz Pineapple juice
* Float Midori melon liqueur

Combine Creme de Banana, Malibu Rum, Pineapple Juice & ice in mixing tin. Shake or flash blend. Strain over ice and float Midori.

Or there's this one. It is sweeter because of the Grenadine.

Monkey in a tree

* 1 1/2 oz Malibu rum
* 1/2 oz Creme de Banana
* 1/8 oz Grenadine
* Fill with Pineapple juice
* Float 1/2 oz Midori melon liqueur

Mix rum, creme de banana, grenadine and pineapple juice. Float Midori on top. Blended and served up is great too.

I've never heard of it either, but guessing by the name, I suspect you are supposed to "float" equal measures of those ingredients one on top of the other, not mix them. Probably Creme in the bottom, Malibu next, Midori on top of that, Pineapple, then sour mix on top.

Just guessing.

no idea...id start with about a shot and a half of each and then add pineapple then taste test from there.
