Initially small branch parts where used, and later refined to using harder materials like rock, to form a very small angle long pyramidal shape - then later surgical steel came into play and better knife edges could be made, which also retained the pyramidal shape along the edge of the blade.

Mythbusters (a show on the Discovery channel) did some experiments on pyramid myths. They didn't do anything with disease, though. They mostly did how fresh things stay.
