it will vary depending on your location. contact the office where you got the original order and ask them, they will likely need your child's school address to add that to the list of places that the respondent is ordered to stay away from.

The same form you used to apply for the original protective order. You can't just 'add' anyone or anything to a protective order. The court has to amend the order.
