A foster parent is one who usually temporarily looks after a child until a 'mother' adopts the child.
A substitute mom is one known these days as someone who will carry a child for another woman who will be it's mother.
When a child is adopted from an orphanage as I know -it is usually for good and with full intention of being as much as a real loving parent , so the woman would thus be a mother in every sense of the word with the exception of having give birth to the child.

i believe it would be caretaker as a foster parent is when a child is placed with a family (mother figure) in their home....since you are specifically saying the child is still in the orphanage, caretaker...
