You'll find it in most of those oriental-type 'supermarkets' usually run by Indians or try one of the really big Japanese places who usually have premises on trading estates.
Otherwise, nag the manager of your local main supermarket to get some in if they haven't already got it on the shelves.

there is a wonderful nearby at Walthamstow, London E.17 where they sell everything including Tapioca flour.
E.4. comes under Chingford, Walthamstow and Highams Park.where there are independant places/shops as well that sell Tapioca flour.
