Yes... or you can just leave it blank.. which a lot of people do. I disagree with leaving it blank though, because if someone gets a hold of your card........... (nuff said) I've noticed a lot of places around here (particularly gas pumps) asking for your ZIP CODE. Not a fool-proof method of course, but it would keep someone from using your card if they found it laying in the street somewhere.

I've had that there for a long time. Please request photo ID. The problem is, probably 70% of the time, they don't. Of course, a lot of times my picture may be on the primary card I use, so this is not as common a problem then. Yes. You can do that.

Yes, but it will make your credit card invalid. For it to be a valid card, it requires a signature on the back.

Even if you write "please see ID," most stores won't. It's your responsibilty to keep your card safe, not theirs.

Not signing your card makes it invalid and a store isn't suppose to take them in that case (not that I've ever come across one who doesn't)
