The son has a set of parents - a father and a mother who is the doctor. The father died and her mother came in and look after her son.

a) the original father in the desert or the doctor are the adopted father of the boy and not the blood relative

You don't actually state whether the Father and son in question are actually related if that makes sense
The man who died could be a father to someone else.
Unless the doctor was grieving the death of the man who died who happened to be their son (Who knows if the doc is man or woman??)
I don't know - I've confused myself even more now!!!! But I think that they are father and son to different people. I think....
