There should be someone at the school or college with the forms but your family income has to be less than £30,000 a year for you to qualify and you have to turn up to every class or you lose your claim for that week.

Your School/College should have a dedicated person administering the EMA, ring the School/College office for their name. They should provide forms and advice.

My friend got one from her local college, if not try your local education authority or the jobcentre (they seem to have everything else).
