Why does he have to ask
If hes so smart wouldnt he know by my behaviour and speech

maybe.. he wants to hear your response. He probably can tell if you've been taking your medication or not.. But he wants to see if you are being truthful.
Hes also trying to evaluate you: by bringing up the medication, you have a chance to tell him what you think of the meds and if you feel the treatment is working or not. After all, it is his job to to be "right on top" of your medical treatment. If the meds don't seem to be working out for you "or" making you worse - it would be his responsibility to make the necessary changes in the meds for the well being of yourself and your therapy.

No, not necessarily. Some people can disguise themselves very good--for short amounts of time.
Over the long-haul, if you're not taking your meds, you are only hurting yourself.
Professionally, every good doctor should ask their patients if they are taking their meds.

Some Meds dont work on everyone and he needs to monitor your porgress in terms of the meds.

Because a lot of people come off them and pretend to be on them to make others feel better.
