Dialect is a separate derivative of a language based on a mother tongue and accent is the way you speak English (or whatever) from your native tongue.
Sicilians in many cases cannot understand many of the words that the Italians in Milan (or anywhere on the mainland) use. As it is a different dialect of the mother language. Their accent is how they inflect and incorporate their language into the speaking of another.

A dialect is an offshoot of a language, such as Mandarin or Cantonese in China. Both are Chinese dialects but are very different from one another.
An accent is the audible effect of one language on another, such as when a person who grew up speaking Mandarin learns later in life to speak in American English.
