I would agree with some of the other answers. Talk to a lawyer first.
I will also tell you about a program that discounts medical / dental services. This program is very inexpensive. I hope it can be of use to you or someone else.

If the dentist says you need a specialist, then you cannot blame him for not wishing to try to fix your problem, and thereby make it worse than it is. So, you either see the specialist, or lose the tooth altogether. It is no one's fault that you have developed periodontal disease, and it is a specialist that has to deal with it. If you want to save your teeth, then that is what has to be done. If you are on a limited budget ask the specialist about financial aid available to dental patients.

I'm' sorry to hear of your unfortunate problem. The only way to correct it would be to remove the crown and re-cement it (the right way) but since he did re-cement it, if he used permanent cement it can't be removed without cutting it off which in return would destroy the crown. If he used a temporary cement, then I would ask to have it redone and cemented correctly. Its a catch 22, you'll have to call the dentist and ask what he used.

First get a lawyer.
Incompetant dentists can be sued for malpractice just like quack doctors. Have your lawyer write a threatening letter telling this ****** dentist to fix his mistakes or pay the penalty.
Then sue his butt.

Try to find an ombuddsman, and a lawyer, maybe even your local member for parliament can help.
