It is pretty standard for these accidents to start happening. She is getting used to you and now...feels comfortable. lol

Start the process of housetraining again and she will pick it up again.
Also clean that area with an enzyme treatment (but don't let her see you cleaning it)

Kennel train. Read my older posts on it, it WORKS. She is just a baby, 12 weeks and totally trained is completely unreasonable and totally unlikely. She's probably confused. Be sure to take her to the exact same spot outside every single time. Don't clean the area perfectly either, leave something for her to smell and think, "Oh, this is where I go!" Also, NO people food. In the home, clean the areas well then clean again with white vinegar and water. Better than Febreeze etc, it takes out the smell as it dries. Repeat if necessary.

Get a small tray or a pile of newspapers and put it wherever you like. Whenever your dog gets in tho the "poop" position pick her up and take her straight to the tray/newspapers then reward her. If she poops on the carpet and you didn't see her doing it, make sure she see's the poop then say NO. Do this everytime so she understands the message your trying to give her.

She is just a baby and expect accidents. Just do not let it go they need to know it is wrong. I never hit my dogs they usually get a stern no, and when I was housebreaking them I took them outside and always highly praised them for doing right. It takes consistency, make sure she gets out frequently, she is still developing and can not hold it as long as an adult dog.

When we first brought my dog home he did that for a couple of weeks but we put a bell on our door so he goes over there and rings it if has to the bathroom try that
