Here are a few suggestions. Instead of trying to give up "chatting" or watching tv, try replacing it with one or more of the following ideas:
-Take up chess, play online, at least 2 games a day
-Start a journal or diary and write down everything that comes to mind. Write every day; make up for days you miss as best you can
-Work on memorizing poems, short stories, a part in a play or scriptures
-Play sports such as baseball, basket ball, soccor, (anything really)
-Get some sleep instead of watching tv or chatting online.
-Drink water instead of soda. Caffein could be keeping you up at night
If you do not get the sleep you need your brain can not function normally. Loss of sleep actually lowers IQ as well.
Any one of these things could help a lot if you stick with it. Taking up two or more things will dramatically improve your concentration. As a USCF chess coach I strongly recommend taking up chess esp since you can always get a game on-line.

Have your parents take you to a School board doctor and get some Ritalin or a newer ADD/ADHD drug, It will do wonders!!!!!!!
OH and if you watch wrestling on TV, STOP.
oh and stop masturbating for a whole month. That will throw your psyche off so bad that you will have an abnormally acute attention span.

When you try to concentrate on reading, try speaking it aloud to follow word by word.
Better still, write it down as you read. This will put extra force on your concentration.
Switch off radios, TV, computers, etc while concentrating.
You can learn some form of meditation to still your mind.

Change your routine as follows:
sleep early and wake up early, this will replenish your lost freshness of mind and soul.
prepare time table and restrict your hours of chatting to not more than 1 hour.
make your studies not a burden on you but enjoy learning and reading books if not your school books then at least newspapers and knowledgeable magazines.

NOPE.so whats with the stop smoking advice....
anyway i suggest you know why your like that..youve said it in ur question....the tv watching etc stop all that..or reduce it
eat healthy food, have a 20min exc daily also read books....do something that works your brain....get atleast 8 hours sleep at night......if i was you id google it...

try to read something
story books or ur school books
get to chat wid ur frnds thru phone or a walk in the evening
get disconnected thru net
solve maths and reading and writing will get ur attention far away from anything

I've heard that this is one of the few positive uses of smoking tobacco--it helps one to concentrate. I've found it to be true.
