Are there any bad side affects to taking creatine ? Have they been proven?

You may assume the creatine isn't spiked with anything else.

Is it ok if I stay on it for three months then come-off it for another three and so on....?

Creatine can cause dehydration due to increased absorption of water by the skeletal muscles and is therefore not recommended those with kidney disease. Otherwise it is ok at 3-5 grams per day if you keep up hydration...i.e drink tons of water.

Taking a "loading" of creatine has not been proven to have any benefit over starting with low doses...so better start with low doses!

Taking those supplements implies that you are also shutting down the natural production of creatine...so take breaks in between...like take it for one month then come-off it for another and so on..

Creatine can cause cramps particularly in the calves, excessive sweating leading to dehydration. The negative side effects far outweigh the the very small gains you'll get taking it.

As long as its pure, you're fine, its only what your body uses in part to build muscle. Just don't take too much if your not going to work out, your body won't have any use for it.
