How old is your friend? Estradiol, which is Estrogen, is a hormone and yes it would cause weight gain but not in specific areas of the body. The weight gain may be from water retention and will be short lived. I wouldn't consider it a safe or healthy way to change a person's appearence and it does carry serious side effects. Your friend might be better off trying something else, like accepting herself as she is.

There is far too little information contained in this question to be able to give a cogent answer. How on earth did she get a prescription for transdermal estradiol without a physician being involved in the transaction? If a doctor was involved, what was it given for? What age is your friend? Has she gone through puberty? How low was her weight? What is her hormone status? Does she have any underlying medical conditions or problems?

Estradiol is the most prescribed "medication" today. If it were intrinsically harmful, in and of itself, it probably would not enjoy the popularity it does, but, like any drug, can be abused.
