Go to the City Hall of Records where you were born. They will have a copy of it.

assuming that you can't ask your parents, check with your local courthouse or government building. also depending on the state that you live in they might have 'open records' law and you may be even able access them online too.

you need to know where you born the county,state etc.. you should be able to go to the couthouse and obtain that info.
