They will start to die right away and will continue to do so for up to 10 days. Some are tougher than others. I hope you went to the vet to have this done. The over the counter ones are not the safest.

Depends on the type of worms. If they are tapeworms (the kind that look like grains of rice in the feces) you need to make sure that what you give your puppy is specifically for tapeworms and not just hooks, whips, and rounds -- which can only be seen under the microscope. Tapeworms come from fleas, so if this is the type of worms you are dealing with, you should consider treating your pet and home (in and out) for fleas.
Generally you need to deworm again one to two more times at two to three week intervals to kill any that the previous treatment did not.

My puppy had to go through two rounds of deworming. We saw worms in the poo after both rounds.

I've seen roundworms come out within minutes squirming like live spaghetti (gross I know). It depends on A) If the puppy actually has worms. B) The type of worm C) How many they have.(if not many you might not even see them)
