Make a day of it... & take her to lunch....
Shheesh... I should forward this response onto my kids...
I would be happy if they just did the lunch part with me...
spending time & enjoying myself with them always means more
me then a gift...
(Ok... So, I still cherish those gifts of spaghetti necklaces made by lil' fingers years ago!)

Asking your mom what she wants or needs is certainly one way to go. Some moms will give a helpful response to that. Others won't and will say something like "oh, nothing ...".
If you have an "oh, nothing" kind of mom, you may have to do some detective work:
What kind of hobbies does she have? Some hobby supplies or a gift certificate to her favorite hobby store might hit the spot.
If she doesn't have any hobbies, what does she do in her spare time when she's not cooking, cleaning, working, etc.? That might give you some ideas for a birthday present.
Does she like clothing? If so, what colors does she wear or look good in? If you have the type of mom who wears scarves, the birthday present could be as easy as picking up a silk scarf in her colors.
If she really enjoys movies but only watches them once, a gift card to a rental place or subscription like Netflix might be thoughtful.
Some women enjoy luxuries like massages, manicures, a day at the spa. However, these can be expensive, and they aren't every female's style.
If none of this sounds like a good fit, there's always flowers and/or dinner out. If you're on a severe budget, fairly nice flowers can be had inexpensively at membership warehouses like CostCo or from your local grocery store.
Things not to get her: pots and pans, perfume or cosmetics (unless you specifically know what kind she uses or wants), garbage disposals or tools (unless she has a hobby like woodworking or car repair).

Just casually ask her what she would like for her birthday. She may stammer a bit about it but any normal mom will let you know what she would like. If she asks you why, tell her the truth that she seems to be very hard to please and in the worst way you want to insure that she is pleased with what you get her.

Take her out to eat and spend the evening with her. She'll appreciate that more than anything. Take her to her favorite restaurant, then go to a movie or go for a walk in the park. Mom's love to get one on one time with their kids, especially after they are starting to grow up and don't have so much time for Mom anymore.

Just ask her. Say, "Mom, what do you want for you birthday?!"
Have her give you a few suggestions so she won't know exactly what you are giving her.
Does she read? Does she like to cook? Does she have a favorite sports team? Would she rather just like to get taken out to dinner by you?
If you would rather not ask her, think carefully about her interests.

Do not ask your mom, she will probably be tickled that you are wanting to buy her something, but tell you, 'Oh no, you don't have to do that!" I like the manicure pedicure idea, if she's not that type of mom, then think about what kinds of things she's bought, or other people have bought for her that make her happy.

Write her a letter telling her how much you love her and how if you could have picked your mother when you were born you would have chosen her. Then give her something that is sentimental like a plate that says world's best mom or something like this. God bless you! You are a wonderful blessing for your mother!

You can simply ask her straight out "Mom, what would you like for your birthday?" or "Is there anything special I can get for your birthday?"
Some gift ideas....A movie gift card. Like AMC and take her to the movies, or if you don't drive she can take you but you pay. Get her popcorn and stuff.
Gift card to her favorite shopping place.
Gas card.

I think, depending on how much money you have, a nice manicure and or pedicure or go all out and get her a massage from a spa! She will love all three of these! Spoil her!
