In Mexico, "not having a grandmother" is being a shameless person, or having no conscience. For example: Juan: "Pedro no tiene abuela. Sale con mujeres nomás para bajarles lana". ("Pedro has no shame. He dates women just to take money from them"). There is an even more common expression used for the same purpose: "No tiene madre". (Doesn't have a mother).
