Is this just an expression? I hear other people say it, threaten whatever, and it sounds like a very very abusive spanking, I would think one would need to be hospitalized in that case.

No, it's just an expression. Most girls, I've found, can sit down almost at once even after a really long hard spanking - though fairly cautiously and maybe with a cushion or two under them.

It is an expression used when something is done to (usually) the butt which is more painful than (normal) or what they are used to; for example: "My hemorrhoids hurt so bad after sitting on the cold cement I couldn't sit down for a week" or "after the birth of my child I had so many stitches I couldn't sit down for a week" or "my Grandmother was so mad when I came home an hour late that she wailed the tar out of me". I have heard of torture where a person is forced to squat in a small box for a week or so causing them to pass out from pain or what is known as sensory supersaturation....i suspect people use this saying to mean they have been somehow tortured but not necessarily in that way. You need to study torture/Coercion/Duress in a decent library. If you can find the following phrase "In The Distance i heard A Scream...It was not till later I realized the scream was my own". Then you have found the right library to study this question.
Most expression have their roots in real life events.

oh c'mon, it's just an expression. an expression with mildly violent undertones. different people do different things, and as long as they take care of what belongs to them, it's not for anyone to judge.
