Each topic mentioned should be a paragraph,

Classes and Grades should be relatively easy as you know what your classes are and don't you want to work towards the best grade you can get? Show how you aim to achieve them.

Standardized tests, these are tests everyone must take, you can talk about how you feel about them, do they make you nervous, do you know the subjects they will cover and so on.

College planning, you didn't say what grade you are in but must be high school, if you didn't think about what college you want to go to maybe this is a great opportunity, think of the career you are going for and research which college offers the best education for this. Even if you don't think you will make it to college one can always dream, but I think if you work hard you will get there.

Career Research should follow whatever you have mentioned in College planning and do the research for that career, how many years of education is required and so forth. Also what the work force future of this career is, there is a great demand for social work careers, health care attendants right now for example.

Portfolio Development, that is your skills, what you have accomplished, being a student you may not think you have alot to put in there but you would be surprised, every job you did you have earned experience, even if not related to your career choice. make a list and a portfolio is something that grows with your experience and compliments your resume when applying for a job, it shows an employer that you are serious in your career choice. PAR stories are great to include in portfolio's you know P. Problem, A. Action taken to solve the problem, and R. Results of the action. Should be short.
