I started over a week ago with a congested nose and bit of a cough and a week later it's still not gone.

Now though when I cough my chest seems to rattle and I cough up horrible stuff. I've just been taking paracetamol but it doesn't seem to be working. I normally try to avoid going to the doctor as not to waste her time, do you thinks it's worth going a week later? What will she do?

Try this, take a clove of garlic, crush it and put it in a pan with water. Bring it to a boil and then remove from the heat lean over the pot, put a towel over your head and breath in the steam. This will loosen up stuff you never knew was in your lungs. If you are still having problems, waste your doctor's time, that's what she's there for!

If you have a fever, or the stuff you are coughing up is rust-colored, then you should see a doctor for the possibility of pneumonia.

If you have a bad cold, it is viral and there really is not much you can do but take care of the symptoms. Try over the counter cold remedies, get plenty of rest and good nutrition. The cold will pass.

What you need to do is drink a lot of pure water, as much as you can do. This will thin your secretions, and make it easier to cough up. Try to up your vitamin C, and go to a doctor for a prescription to treat the infection.

Go to the doctor's. Paracetamol is definitely not enough if you're coughing up anything that's from green to brown in colour and thick. You need antibiotics.

I'm a nurse. Please go to the doctor and get treated. It sounds very much like you have an infection, one that is easily spread to other people. You will not be wasting your doctor's time. Go before it gets harder to breathe.
