Several possible reasons:
1) Your oven heat is uneven. Solution: turn the bread from time to time during baking.
2) Your dough is unevenly distributed in the pan. Solution: place a folded towel on the table and gently drop the pan onto it a few times. (Works to remove air bubbles from cake batter, too.)
3) The yeast in your dough is unevenly distributed. Solution: don't literally punch down your dough after the first rise; just sort of fold it over and flatten it into itself so that all the new yeast cells are nicely spread out before the final rise.

This could be due to uneven heat distribution in the oven. My oven is gas fired so the dough in the tin gets hotter nearer the flame making it rise more that side as the yeast works more actively in the warmer part of the loaf before it gets too hot and dies.
I compensate for this by rotating the loaf tin around about 10 or 15 minutes into the baking and it seems to work. It also ensures a more even colour on the crust of the bread.

This is usually caused by an uneven heat in the oven. This can be caused by faulty equipment (element, burner) or by a door that doesn't close perfectly.
One way to at least partially deal with this (until the equipment is repaired, replaced):
1- place a water-filled dish (oven-proof, of course) in the bottom of the oven when you turn the oven on;
2- preheat your oven VERY high before the bread goes in; turn it down to baking temperature after 10 minutes.
The steamy interior of the oven will allow a more even distribution of heat and the high heat will make the bread rise and "seize" more quickly.

probably you didn't pour your batter evenly, try lightly slamming the bread pan on the counter a few times to make it even. That's what my mom taught me when making a cake.
