Good sleep, good diet, decreasing caffiene and sugar in the diet, relaxation and exercise, walking, talking to a friend, taking a break then going back to studying just even getting up for a drink of water or a bite of healthy food can help. Switching between books helped me as they were different subjects thus using different parts of my brain. Eating fish or taking Omega Fatty acids have been shown to help but not in excess. I prefer to eat fish as this helps me also. My brother in law said to me when I get tired of studying I take a break and do something I hate like house keeping so I will go back to it fast. My friend said to me the reason you get so tired and your mind wanders is you are studying a hard subject if all you can study is five minutes get up take a break do something come back to it really fast that little break will help that is how I got my sociology psychology degree.

I can give you many advice like other people. But I know well that the effects of advice won't last much longer. Your problem may be due to some other diseases. Stress, Depression, Anxiety all can cause problems like yours. So please try to consult with a Clinical Psychiatrist and to find the underlying cause.
Drugs like Ritalin, Modafinil and Bupropion can increase focus and concentration. Bupropion (Zyban or wellbutrin) is the safest of them.

take foods that are high in omega 3 fish oils these help over time to improve concentration, but in meantime take plenty of breaks in between studying an get some fresh air also

Ermm.....avoid wanking, avoid caffeine by any cause, its a stimulant yes, but once u get ' immune' to the effects of caffeine, you get anxiety, and sometimes, bad memory and focus, sure anyone will tell you its enriched with thousands of antioxidant, but its actually a pernicious neurotoxin, mainly because of the theobromine element, that is a byproduct of caffeine metabolization, and by all cause, stay the hell away from nootropics or mind-enhancing drugs, especially piracetam, its all a bunch of rip-off, cos it doesnt work, it will probably create a temporary boost, but it has adverse effects. thirdly, try meditation,

Have enough sleep, eat healthy, avoid mind altering drugs and alcohol, get rid of any distractions when studying..... focus on ur goals....
and once in a while go to YA to exercise your brain cells....but avoid useless chats esp on weekdays.....Good luck in ur studies.
