Yes, this is commonly called a "live straddle". A player may only post a "live straddle" when he is directly to the left of the big blind (or "under the gun"). A live straddle means that he posts two big blinds, (i.e.: in a 5/10 game, a live straddle would be $10) and is the last person to act preflop. Generally, when a player straddles, it does not count as a raise, so in four-bet/raise maximum games, with a live straddle you increase to 5-bets. I.E. in a 3/6 game, normally the action would be capped at $12, with a straddle, it could go as high as $15. After the preflop action the play resumes to normal, that is, the first person to act is the first person that still has cards and is left of the dealer button.

It's a house rule and varies from place to place. Most live casinos will let you straddle. I've never, ever seen an online site let you do so (probably because it's a bit of a rare thing to happen and they don't want to confuse new players).
I'd avoid it. Anytime you have to pay more blinds is bad. You are paying more for the worst position at the table, which can't be smart.

Dont think so, Even if small big blind goes all in and loses, The blind get moved forward with 2 big blind next hand,No one gets to miss the blind.

Yes it is done all the time, usually in higher end cash games, just remember all players at the table must agree to it.
