You will need to submit a letter to the Church Records Division the address and a form letter is available at http://www.mormonnomore.com/
It may not be an easy process though. The Records Division is known for returning letters, telling you falsely that you have to submit it by hand to your bishop. This is NOT true. It is a ploy to force you into an interview with the bishop. Many times the church has returned letters and threatened excommunication. THIS IS AGAINST THE LAW!!! They cannot threaten you with any legal or socially destructive action! Many people have reported that the church has contacted their bishop or ward president and family members. Many wards will come to your home, send out missionaries and recruit your TBM (True Believing Mormon) friends and neighbors to help you "rethink" what you are about to do. Many members have found that their familes have been encouraged to shun them, and familes have suffered over this. Be strong and be brave. This is not an easy thing to do, but it will be worth it. If you have found out the truth about the so-called church (and if you want to resign you must have) than you will not be able to just sit idly by and remain a member. Good luck!

They will take you off, this is what you need to write:

This letter is my formal resignation from the Church of Jesus Christ of Latter-day Saints and it is effective immediately. I hereby withdraw my consent to being treated as a member and I withdraw my consent to being subject to church rules, policies, beliefs and 'discipline'. As I am no longer a member, I want my name permanently and completely removed from the membership rolls of the church.
I have given this matter considerable thought. I understand what you consider the 'seriousness' and the 'consequences' of my actions. I'm glad that you now believe that the Mormon church is not right for you and your family. I hope you also believe that Jesus is the One True Way and have confessed your sins to Him and asked for His forgiveness. I also hope that you start attending a church where the Bible is preached in it's wholeness and completeness, not just "as long as it is translated correctly" (Article of Confessions - Mormon church).
Unfortunately, they will probably never officially take you off their registers. The guy who suggested you write a certified letter has a point, but only if you want to go to that trouble. You could also just stop attending and firmly, consistently and gently tell them that you have no intentions of going back. Good luck!

It's very hard. It's like trying to get yourself removed from Republican Party fund-rasing lists. You're talking about an organization that does baptism by proxy for the dead, so do you think they really care who does or doesn't want to be on the membership roles, unless they want to remove you for their own reasons? You might find the site below of some use.

Hmmm, well as a fellow member of the church, I totally disagree with you. It's a wonderful church full of wonderful people, and it brings me alot of happiness. How can a church that encourages its members to prayerfully study every principle to determine for themselves whether or not its true be "brainwashing" people?

Keep a copy.
THEY WILL NEVER NEVER take you off their lists.
