No, some times people literally don't understand and need clarification.

No, not always. A better phrase for them to say would be "I don't understand what you are saying." Since I don't know what you mean by this, I am only guessing. That is something has been said to me many times by my mother, but was usually finished by, "you won't listen and do try to do everything you shouldn't do." Would you mind editing your question.

it might but it is more one of the following :
1. language barrier - he might not understand the accent, dialect or some of the native words of the speaker.
2. dyslexic or some inherent problem in speaking like incoherent thoughts, random blurts
3. Inaudible - due to heavy surrounding noise, or faulty equipment (if speaking on mobile or handset)
4. Not looking eye to eye - two people not finding common ground
5. Pretention - sometimes as you say some people have to pretend to get away from the situation. now that situation is something everyone hates to be in.

I think it depends on the spirit in which the statement is said. "I don't understand you" could mean, "I've tried, but I give up now", or "I see that we're not on the same wavelength, can we find some common ground"....and so on...
But I think generally, someone actually taking the effort to say "I don't understand you" instead of just tuning them out or walking away, indicates that they would LIKE to understand, but are not succeeding at it. =)

Not necessarily. I don't understand some people but does not mean I don't want to. Some are just a lot more challenging than others.

Yes, It can mean that or it could mean I don't want to tell you I understand, I wasn't listening, and maybe just I don't understand. That's why we have intuition to know.

Yes I do believe that to be true, usually it comes from people with no depth of character. " I don't understand you" means "I think your stupid for thinking that or acting that way" I say... dump them, you deserve better and rock on baby.... you must be a stellar person!

I don't think so. I think a lot of times it means what it means. So much of our thought life and epxerience is internal and inaccessible, even to us, that it's not surprising our actions and thoughts would be incoherent to someone who has no context to udnertstand them in. It's probably more miraculous that we are ever understood at all.

sometimes it means that they just dont understand you and need further clairfication but sometimes it means that the person is not listening hard enough it can go either way

It depends on the quality of truth that you want them to understand or if it is really falseness.

Also -

Some people prefer what is false to what is true and some people prefer what is true over false.
