I think there has always been a nursing shortage. No mater how many are unemployed, doesn't mean there is not a shortage of employed nurses. Why? Working conditions, due to a shortage of nurses. Stress.

There's a shortage of nurses overseas when people in these countries found out that nursing isn't a "well-protected" job - nurses are exposed to all sorts of contagious diseases and I think there's hardly any benefit or perk available for a nurse who gets contaminated by a patient. Plus - the actual service rendered by nurses - the yucky part - like cleaning bed pans and washing the genitals of handicapped people - do not appeal to some people in these countries. To most americans and europeans - it is a service that's "below" their status in life. Some maids don't even do that for their adult employers. With the coming of AIDS and how easy it is for a nurse to be infected - all the more reason some decided not to become nurses anymore.

When? I don't know but it's been ~5 years ago at least.
How and why? It's a lot like for teachers... Low pay (for what they do), bad working conditions (too much bureaucracy), too many people with bad or bigger than God attitudes (doctors' treatment of nurses), long hours, dealing with too many people dying, and just not enough people wanting to go to school to become nurses. There could also be not ehough people to want to finish nursing school.
