Over here in the Netherlands we use a mixture of 2 parts sugar and 1 part cocoa.
Add to warm milk at flavor and top with whipping cream.

If you want to make it from scratch, here's how to do it:
1. Take 1 Tsp. powdered unsweetened cocoa + 1 Tbs. white sugar and on the bottom of the cup or glass you intend to use, mix dry without adding any liquid.
2. When this is thoroughly mixed, add a little bit of cold milk (water will do too) and make a paste. Be careful not to add too much liquid in the beginning.
2. When you have a thick dark brown paste, add cold or warm milk to fill the cup or glass, stirring constantly.
Play around with the quantities of cocoa and sugar, until it's perfect for your taste.
