You do not necessisarily have to be completely spread out to float. But this technique does increase the surface area that your body is in contact with water. This decreases the pressure your body exerts on the water's surface allowing you to float easier. Your body actaully floats because of the air in your lungs and the oxygen attached to your blood which both increases bouyancy.

Oxygen spread over a larger surface makes you more buoyant. Ever notice when you take a breath in, you float better... and when you exhale, you begin to sink a bit?
