Maybe he thinks that both of you are an 'item' now, and he doesn't need to impress you. That is unfortunate - it could be he is taking you for granted, but you don't want to be.
So discuss it with him and tell him that you still enjoy going out on dates with him because you enjoy spending time with him as a couple. Also give him suggestions about things to do, so he doesn't have to guess. And ask him what he would like to do - you could take the lead and take him on a date.
As for what to wear - find out where your date is and dress up for it - feel good about yourself, and tell him that he should dress appropriately so both of you look good.

I agree with #1... I would be cute and sexy, get him all wound up then take a fake call or get a friend to call, then bail on him.... He's diffenetly got somone on the side...
