Well, if you ever plan to make love to her, you will need to tell her. The other option is to let her find it for herself. Surprises like that are usually not so good. Just tell her.
BTW, the condition is called "polyorchidism".

I did not know that was possible. My son who is 9 says that when he gets bigger he will have 3 and swears he only had one when he was born but grew the 2nd after about 3 yrs old.
