being constantly conscience of your posture. Maybe get a mirror at work if you sit at a desk or write down in big words "posture" where you will see it alot until it becomes a good habit.

I've been more aware of mine lately too. Pretend a string is pulling you up from your head. Works for me. Shoulders back and down. (Then just loosen up a little until you no longer feel like a soldier!) Also, massage is great- it loosens you up and makes it easier to maintain good posture.

I notice people's posture when they walk. Head and shoulders up straight. I think it just takes practice.

What I've done is to slightly straighten the seat in my truck (so it's really not laying back at all) and played with the lumbar support on the seat so that it pushes out pretty far into my back when I'm driving. I slouch really badly when I drive and that combination forces me to keep my back straight. When your back is straight like that, it's pretty difficult to let your shoulders slump.

it helps to wear heels, so that you have to arch your back while walking, and that will get you into the habit since you HAVE to have good posture to balance standing in heels.
