Really, you don't...unless you are peeved at someone's very faulty performance as a human, or as a politician. It is offensive...but sometimes befitting.

After women take over HR in a large company, and establish a system which absolutely discriminates against men, they call the men who can't get jobs because of their deliberate discrimination, losers. Yeah, it's very offensive.
The problem is we need to call female losers, losers. Women who marry stupidly and then divorce and mess their kids up are losers, but as a sexist nation, we don't call them that as we should.
You males will marry better if you learn how to spot female losers.

You don't, unless you are joking with someone that you know will take it as a joke.
It's not as offensive as, say, certain potty-mouth expressions, but it's pretty harsh. You're basically calling the person worthless.
Of course, applied to one's precious own self, it's acceptable.

It does not need to be verbally expressed. It is generally understood by everyone that the individual has little character. Some things are quite clearly understood without explanation.
