If in the US and going through an airport that has TSA, all they will do if they see it on the x-ray is do a bag check and compare your name on your ticket to the name on the insulin. I'm just assuming that you'll have one that the prescription label is still on. I've never asked for a doctors note, I just looked at the name and as long as it matches your good.

Not only do you need the old fashioned doctors notes, but you will also need your ID to compare the name on the prescription, or else you cant take it. They will also examine it to see if it is what it is.
There is usually a place to leave the syringe in the bathroom, please do that and not leave it in the seatback, it can cause injury.
Northwest Airlines empties that container into Biohazard waste every Sunday night.

My son has to carry an injection for other reasons and he has to have a doctors note with him whenever he travels.
