Medicine has become so effectiive at treating so many diseases, and at extending life, that possibly the biggest threat society faces is the impending imbalance in the population. Sooner than you think there will be more elderly & feeble (but still living) individuals in their 80s, 90s and beyond than there are younger, healthy adults to maintain the operation of society. There will be a smaller working tax base paying in to the system, and nore people taking out in the form of retirement benefits. Before too long, people in their 20s, 30s, 40s & 50s will begin to resent paying more than half their income in taxes to support those in their 60s, 70s, 80s, 90s & beyond.
Racism will disappear & be replaced with Ageism.

Nuclear weapon with it's EMP shockwave going off in a major financial center, like London or New York City. This would cause major upheavals in financial markets (not to mention hundreds of thousands of casualties), along with a massive military response. This, in turn, would kill millions and the threat of nuclear holocaust would be very real at this point, further destabilizing the world economy.
The answer to your question is terrorism, not because of the act itself, but because of the cascade effects a successful nuclear strike by terrorists on a major western city would have.

Terrorism and any fanatical thinking that "justifies" killing and creating chaos to further any particular beliefs. Unfortunately this has "trumped" other threats because it is so random and so wicked (the suggestion of an action by a handful of zealots can paralyze entire countries).
Poverty, physically and intellectually. Everyone has the right to healthy food and water, safe shelter, necessary medical aid and basic education.
Pollution which is very much man-made on land, in the air and oceans.
Violence -- war, ethnic "cleansing", rape, sexual exploitation and abuse especially of children, slavery, racism...
This is getting too depressing so I'll stop.

Religion is the greatest man-made threat to our current society...It is causing hatred/violence and it is responsible for not connecting humanity with the true and living God....These other things would evaporate if God's presence was truly manifest in our world.
