If I were to stab you with a fork in the leg, the pain would be on your mind...

If I were to conduct this experiment by stabbing you in the brain with the same fork, it would then be in your mind

On one's mind would mean thoughts that are current.
In one's mind would mean thoughts from the past that are in your subconscious that surface periodically.

On my mind implies that this is uppermost in my thoughts.
In my mind implies that it is one thing among many others.

Think of it this way:

PUT THAT IN MY FILE CABINET.
vs
PUT THAT ON MY FILE CABINET.
